I give permission for my child to travel internationally via Bahamas Air for the dates specified above.*

I confirm my child’s participation in summer camp programming and supervised excursions.*

 I acknowledge the staff and partners responsible for supervision:

• Avenues of Excellence Finishing School

• Cultural Cosmetics, Inc.

• Red Carpet Girl Mentoring Program

• Yellow Mango Productions

• The Start Program

I authorize emergency medical care and transport decisions if I cannot be reached.*

I approve administration of over-the-counter medications (Tylenol, Benadryl, Ibuprofen, etc.).*

I understand that participation includes travel, lodging, recreational, and group activities that involve risk.*

I release all listed organizations and affiliates from liability due to injury, illness (including COVID), or personal loss.*

I agree to hold harmless all listed entities.*

I grant permission for my child’s image and/or voice to be photographed, recorded, or otherwise documented during the program.*

I authorize the use of media for promotion, documentation, or publication by participating organizations.*

I acknowledge the rules and expectations for participants including:

• Curfew compliance

• No alcohol, drugs, or tobacco

• Adherence to dress code

• Respectful behavior toward staff and peers

I understand that violations may result in disciplinary action or removal from the program.*

I confirm that valid travel documents (passport, visa if applicable) have been submitted for my child.*

I understand that failure to submit proper documentation will result in withdrawal from the program.*

I authorize my child to be transported to and from activities by authorized staff or vendors using licensed vehicles including:

• Vans

• Buses

• Car services

• Boats

• Air Craft
